I want to book a follow up appointment with my prescriber. icon

My pharmacy app is not showing my medication as available. icon

Please call your pharmacy to confirm that your medication is not on hold or that your pharmacy is currently out of stock of the medication.

I lost my medication and do not have any more refills at my pharmacy. icon

I want to cancel my upcoming appointment but I can not call during business hours. icon

Leave a voicemail or email us at help@insightchoices.com to cancel your upcoming appointment. Please make sure you cancel at least 24 hours prior to your appointment time. Otherwise, you will be charged the $60 late cancellation fee.

How soon will you get back to me once I have sent an email with a question? icon

Please give us up to 48 hours to get back to you with a response from your provider.

Can I book an appointment without having to pay in advance? icon

If you request an appointment using our Patient Portal, no payment is collected when you request the appointment. We will charge your credit card on file for the cost share when we process your appointment request. If your credit card on file declines the payment, then we will call you and collect the payment over the phone before confirming the appointment. If we cannot collect the payment from you, then your appointment request will be cancelled due to nonpayment.  If you call to book an appointment, we will ask to collect your cost share before we can finalize the booking of your appointment. We cannot confirm your appointment without cost share payment from you in advance.  

How much will the cost be out of pocket to see my provider? icon

If you don’t use your insurance, then the discounted self-pay fee for medication management follow up visit is $150. New patient appointments for medication management is $220. Therapy follow up visit is $120. New patient therapy appointment is $160. If you use your insurance and deductible applies, then the allowable rate based on diagnosis, procedure code, provider category, region, and provider contract is how much you will need to pay. The allowable rate can be higher or lower than the discounted self-pay fee, depending on the insurance company.

I am a new patient. How long is the new patient intake process? icon

Once we receive your information via text or email please give us 24 business hours to get back to you and proceed with the next step of the intake process.

My medication needs Prior authorization. How can I get one done? icon

Please contact us so that we can assist you with your prior authorization. Please make sure that we have the most up to date information (current insurance, current address) on your file to complete your prior authorization.

What is HSA and HRA, and does Insight Choices accept it as a form of payment? icon

HSA, or Health Savings Account,  is a type of personal savings account you can set up to pay certain health care costs. An HSA allows you to put money away and withdraw it tax-free, as long as you use it for qualified medical expenses, like deductibles, co-payments, coinsurance, and more.  Insight Choices accepts this as a method of payment for service, just as we would a major credit card or other debit card. 

HRA, or Health Reimbursement Arrangement/Account, is an employer-owned and funded account. Only your employer can contribute money to your HRA. You can use this account to reimburse eligible medical expenses.  Insight Choices accepts this as a method of payment for services, just as we would an insurance payment.

Please let your scheduling coordinator know if you have HRA or HSA, as well as if you have a savings account debit card that you would like to use for your cost share responsibility.

How do I access my HSA money? icon

Your HSA will provide you with a debit card to spend your HSA funds. You may use this debit card as a form of payment for your office visits.

Your employer decides how you’ll access your HRA, but it will likely be one of the following methods:

  • You won’t do anything — most plans will reimburse your network doctor directly
  • You’ll use a debit card tied to the account, if offered by your employer
  • You’ll pay for expenses up front, then request reimbursement from the employer.
